Liability Waiver and Release

In consideration of being allowed to participate in sports camp activities at Capistrano Valley High School, I, for myself and my participant, agree to the following:

— Assumption of Risk: I understand that participating in these activities involves inherent risks, including the potential for serious injury, disability, or death, and I voluntarily assume all such risks.
— Release of Liability: I release and hold harmless Capistrano Valley High School, its coaches, employees, contractors, and agents from any claims, demands, or liability for injury, death, or property damage, including claims arising from the negligence of the released parties.
— Medical Consent: I authorize necessary emergency medical treatment in the event of injury or illness and accept financial responsibility for any associated costs.
